China

Makes sense when: the case is a cell therapy (CAR-T, TIL) or a registered trial where China has active research, and eligibility has been confirmed in writing by the treating team.
Does not make sense when: standard treatment is already available locally, or eligibility has not been confirmed in writing.
Practical barrier: medical visa, substantial prepayment, and language support vary by hospital.

USA

Makes sense when: the case needs a newly approved drug or a registered trial not available elsewhere, and insurance or funding has been confirmed.
Does not make sense when: the same standard treatment exists locally, or the cost is uncertain and no funding plan is in place.
Practical barrier: B-2 visa, very high out-of-pocket cost, and insurance that rarely covers non-residents.

India

Makes sense when: the goal is standard oncology care at a lower cost, and the hospital has given a written quote for the specific case.
Does not make sense when: the case needs a therapy that is only available in a research setting, or the quote has not been verified in writing.
Practical barrier: quality varies widely between hospitals, and medical visa processing can take time.

Turkey

Makes sense when: the case is a standard treatment with a shorter travel distance, and the hospital has confirmed the plan and the quote.
Does not make sense when: the case depends on a therapy that is not routinely offered, or travel safety has not been cleared by the treating oncologist.
Practical barrier: language support and interpreter availability vary by hospital; visa rules depend on nationality.

Germany

Makes sense when: the case needs a specific protocol, a formal second opinion, or a trial well matched to the diagnosis.
Does not make sense when: the same standard care is available locally, or the case has not yet been reviewed by a multi-disciplinary team.
Practical barrier: Schengen visa, high cost, and long waiting times for non-urgent reviews.

I won't pretend we have no financial interest: hospitals pay us a percentage when a patient chooses them. That is exactly why we publish our criteria, why we list more than one option, and why we say "don't go" when travel is not the right next step. Trust comes from admitting the conflict, not denying it.
