Complete the form with your name, contact information, country of residence, and preferred destination. No medical data is collected.

We Forward Your Contact Information

With your explicit consent, your name and contact details are forwarded to one or more partner centers in your preferred destination country.

Partner Center Contacts You Directly

The partner treatment center or medical tourism company contacts you directly. From this point, all communication is between you and them.

Medical Assessment by Licensed Physicians

The center's physicians review your medical records, make diagnoses, and discuss treatment options directly with you.

Treatment & Follow-up
